*Program Fees: I acknowledge the fees associated with the program and agree to pay them.

*Emergency Contact Information: Agreement to provide reliable emergency contact details.

Medical Consent: Permission for the program to seek medical treatment for the child in case of an emergency.

*Medication Policy : I understand that if my child requires medication, a completed medication form must be submitted.

*Program Hours: I understand that program hours are: Monday–Thursday: 9:00 AM – 3:00 PM   Friday: 9:00 AM – 12:30 PM

*Pick-Up/Drop-Off Procedures: I agree to follow the program’s procedures and times for pick-up and drop-off of participants. Late pick ups may be subject to a late charge.

*Contact Information Accuracy : I confirm that all contact information provided is accurate and up-to-date.

*Emergency Contact Commitment: I agree to provide reliable emergency contact information that can be reached during program hours.

*Liability Waiver: Parents understand and accept any inherent risks associated with the program and release the organization from liability.

*Pick-up/Drop-off Procedures: Agreement to adhere to the program's procedures for pick-up and drop-off of participants.

*Deposit: I acknowledge that a $50 deposit is required for registration to be complete and will be refunded upon program conclusion or in your final registered week

*Food Responsibility: I understand that I am responsible for providing snacks and lunches for my child.

*Temporary Program Notice: I understand that this is a temporary program available only during the teacher strike, and programming will cease when school resumes.
